- High School Diploma certificates as provided by the issuing Institution (10th, 12th).
- Degree qualification certificates and transcript(s) as provided by the issuing institution (BDS, DDS).
- Official translation in English language of the above- mentioned qualifications (if any).
- International English language certificate (if any)

- Consent form
- Dental registration certificate.
- Receipt of payment of first instalment of the application fee.
